We are excited to welcome new students and families to Washington School! Please review the important pre-registration information below to ensure a smooth enrollment process.
Pre-Registration Requirements
Please complete the required information for each child entering Kindergarten, 1st, 2nd, 3rd Grade. Complete this FORM.
If your child is a current student at Washington School, you do NOT need to complete this form.
Registration and payment for current students will begin in July.
Health Requirements
To meet Illinois state requirements, please schedule your child’s appointments as soon as possible:
Physical Exam (including updated immunizations)
Dental Exam
Eye Examination

Important Deadline:
The physical exam (with updated immunizations) and vision forms must be submitted by September 1, 2026.
Birth Certificate Requirement
A certified birth certificate (with a raised seal) must be submitted to the Washington School Office.
